- order repeat medication
- nominate a pharmacy
- view medical records

We have a new way in which you can contact us online for any routine enquries. We will get back to you within one working day. You can use this system to:
- Request sick notes
- Request letters
- Submit a query about prescriptions
- Ask for advice about non urgent medical conditions

PROXY Access
Proxy Access
As a parent, family member or carer, you may be able to access services for someone else using linked profiles. This is also sometimes called having proxy access. It needs to be set up by the GP surgery where you and the other person are both registered.
If you would like to set up proxy access, please contact the surgery.
Enhanced access
Starting from July 1, 2025, Middleton PCN extended access services will be held at Woodside Medical Centre

What This Means for You
- Extended access hours will remain the same. Weekdays 6:30pm to 8:00pm and Saturday 9:00am to 5:00pm
- Services currently offered in Extended Access will continue to be offered at Woodside Medical Centre.
- For any questions or concerns, please contact your GP surgery.
